Responsibilities The Speech Language Pathologist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. This may be a patient’s private residence or a room in an assisted or independent living facility. The role involves evaluating and treating patients in the most functional environment—their home—while coordinating care within a multidisciplinary team and maintaining a safe, compassionate environment. Supervise CFYs temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards. Adhere to the department and patient treatment schedule, timely clinical and billing documentation, and work‑related meetings. Attend and contribute to patient care, staffing conferences, and related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult with rehabilitation team members and the physician to select the most appropriate therapeutic regimen based on patient needs and capabilities. Adapt program and/or plan of care according to individual patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report need for repair of equipment. Prioritize patient service, ensuring residents and families receive the highest quality of service in a caring atmosphere that recognizes individual needs and rights. Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures, synthesize information, interpret findings to establish diagnosis, identify impairments, determine predicted improvement and time to achieve it, identify precautions/contraindications, and design a plan of care. Develop treatment goals and methods in collaboration with patient and caregivers, implement the treatment plan, and complete all related documentation. Initiate and coordinate the acquisition of adaptive communication devices and instruct patients in their use. Facilitate identification of hearing deficits and refer appropriately for assessment, diagnosis, rehabilitation, and/or amplification. Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interests. Instruct patients in skills and techniques required for therapeutic activities, evaluating progress, attitudes, and behavior relative to rehabilitative potential. Perform routine reexamination as needed to modify or progress the treatment plan. Coordinate home and community care with other interdisciplinary team members in collaboration with the referring physician. Perform other related duties as required.
