Geriatric APP/PA: Primary Care with Home Visits

Element Care

Element Care is seeking an Advanced Practice Provider (NP/PA) to join the PACE Interdisciplinary Team delivering primary care to participants in Massachusetts. The APP will conduct comprehensive physicals, evaluate participants in various settings, and provide home and center-based care as needed. Must hold MSN or PA studies, be board-certified in MA, and possess MA and DEA privileges with CPR certification. #J-18808-Ljbffr Element Care
