Occupational Therapist (OT) - Post-Acute Rehab (SNF) - PRN
As a PRN Occupational Therapist, you will evaluate and treat patients with challenges related to mobility, cognition, strength, and activities of daily living. You'll collaborate with interdisciplinary teams to deliver individualized, patient-centered care in accordance with AOTA standards.
Key Responsibilities
- Evaluate patients within 2448 hours of physician orders
- Develop and implement individualized treatment plans
- Supervise OTAs and support care delivery
- Communicate with physicians, care teams, patients, and families
- Document care, progress, and billing accurately
- Participate in care planning and discharge processes
- Provide caregiver and staff training
- Recommend adaptive equipment to support patient outcomes

Qualifications
- Bachelor's, Master's, or Doctorate in Occupational Therapy
- Active Occupational Therapist license (state-specific)
