Job Duties:

• Conduct nutritional assessments and develop individualized care plans
• Monitor weight changes and manage tube feedings
• Complete MDS and CAA documentation and participate in discharge planning
• Collaborate with nursing, therapy, and administration teams
• Provide diet counseling and attend resident council meetings
• Conduct audits, supervise dietetic technicians, and deliver staff training
• Revise menus and ensure dietary compliance with facility standards

Qualifications:

• Bachelor's degree in Dietetics
• Registered with the Commission on Dietetic Registration
• Licensed by the Ohio Board of Dietetics
• Minimum 2 years' experience in geriatric care
• Strong knowledge of long-term care regulations and MDS process
• Excellent communication, computer, and decision-making skills
• Ability to travel between facilities and perform inspections
