Advanced Nurse Practitioner (MSN, RN)

Position Overview

This role is dedicated to delivering clinical and palliative care services within hospital or homecare environments. The practitioner supports the assessment, development, implementation, monitoring, and evaluation of clinical programs and policies. They actively contribute to quality improvement initiatives and collaborate closely with physicians and healthcare teams to optimize patient outcomes and resource management.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
• Provide comprehensive clinical services in hospital settings.
• Monitor patient outcomes, identify barriers to care, and collaborate with interdisciplinary teams to address challenges.
• Educate physicians, inpatient staff, and community members on hospice and end-of-life care services.
• Participate actively in performance improvement programs.
• Oversee clinical service assessments, development, and evaluations.
• Conduct patient health assessments through histories and physical exams.
• Refer patients to alternate care options as needed.
• Coordinate with Infection Control for reporting, screening, and prevention programs related to infectious diseases.
• Prepare and present monthly reports and statistics to management; attend relevant meetings and workshops.
• Develop and review clinical policies, procedures, and monitoring tools.
• Continuously improve quality of care and service delivery.
• Collaborate with medical directors, physicians, and healthcare professionals to ensure effective clinical service provision.

Qualifications

• Minimum of 2 years experience as an adult Nurse Practitioner preferred; candidates with less NP experience but at least 5 years of RN acute care experience will be considered.
• Completion of all Nurse Practitioner clinical requirements.
• Proficient with personal computers.
• Excellent communication and customer service skills.
• Bilingual in English and Spanish is strongly preferred.

Education

• Graduate of an accredited nursing school.
• Master of Science in Nursing (MSN) degree required.

Licenses and Certifications

• Current Registered Nurse (RN) license.
• Current Nurse Practitioner license.
• Basic Life Support (BLS) certification from the American Heart Association required upon hire.

Employment is contingent upon passing a physical exam, drug screening, and background checks prior to start date. This position is located in a regional healthcare setting within the Pacific Southwest region of the United States.
