Job Summary: This position is responsible for providing clients and family members with support and information with Bethesda Home Health and Bethesda Hospice. This role provides the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. This position oversees and coordinates the admission process. The social worker serves as the client and family liaison. This position oversees the grievance process and is responsible for thorough follow-up of all concerns. This position assists the director with overseeing the quality assurance program, ensuring that high quality care and services are being provided to the clients of Bethesda Home Health and Bethesda Hospice and clients and their families are satisfied with the care and services provided. Other duties include managing the organization's CRM platform.

Job Relationships:

• Reports to: Director of Home Health and Hospice
• Interrelationship with: MSW, Hospice Clinical Nurse Manager, Staffing Coordinator, RNs, LPN, and entire home health and hospice team, Welcome Center, Chaplains, Administration

C. External Contacts: Visitors, families, other health service providers, potential clients and families, other servicing agencies

Hours of Work: Salaried/exempt position – typically Mon. – Fri., 8:00 AM to 4:30 PM. Extended/additional hours as needed.

Dress Code: Business casual

Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity)

A. Education and Training:

1. A bachelor's degree in a human services field including, but not limited to, sociology, gerontology, social work, human services, healthcare administration, and psychology.

2. Must be licensed by the state of Minnesota.

1. Experience: Previous social work or gerontology experience is desirable.

D. Job Knowledge (Specific)

• Knowledge of social work practice.
• Knowledge of the health care delivery system and broad base knowledge of community/state services available for referral.
• Working knowledge of general medical problems of the geriatric disease process, required for accurate assessment and evaluation of incoming clients.
• Knowledge of the legal system as it pertains to the protection of vulnerable adults and preservation of rights under law.
• Knowledge of the assistance system and programs developed to provide financial assistance.

Physical Demands of the Job:

• Light duty position with prolonged periods of sitting and walking.
• Prolonged periods of using computer keyboard/laptop

Principle Job Responsibilities:

Responsibility: Deliver professional and thorough social work services, including psychosocial assessment and intervention planning.

1. Identify patients who require social work assessment and intervention through high-risk screening, interdisciplinary team meetings and individual referrals.
2. Conducts assessments and completed individualized plans of care.
3. Provides support and counseling to clients and families.
4. Provides information and assistance for identified financial or social needs.

Responsibility: Communicate with potential referrals and oversee the referral process with Admissions Coordinator.

1. Provide information to prospective clients and family members.
2. Return calls and answer questions for potential clients and family members.
3. Manage the organization's CRM platform.
4. Receive information from referral sources.
5. Gather required information about client to determine admissions.
6. Communicate prospective resident information with clinical team.
7. Explain the services to potential referrals.

Responsibility: Market and promote Home Health and Hospice services.

1. Maintain ongoing liaison with staff, clients, family members, and community.
2. Work with hospital discharge planners, nursing home social workers, county social workers, and physicians to determine appropriate referrals.
3. Assure accuracy of public information materials and activities. Promote services at health fairs and other community events as appropriate.
4. Coordinate with community director to plan and schedule community events to promote home care services and generate referrals.

Responsibility: Serve as the primary contact for client services outside of clinical questions and manage the grievance/VA process.

1. Serve as the client and family liaison.
2. Receive and track grievances through their conclusion.
3. Communicate grievance decisions to client and any other applicable parties.
4. Immediately report all alleged violations involving neglect, abuse, injuries of unknown origin, and/or misappropriation to the Director of Home Health.
5. Ensure that grievance decisions include the required documentation and are saved for the required amount of time.

Responsibility: Assure quality client care.

1. Assist with development, implementation, review, and revising of policies and procedure that guide and support the provision of services.
2. Assist with assurance compliance with State and Federal regulatory requirements.
3. Assist with development, implementation, review and revising of a Quality Assurance/Improvement Program
4. Track statistical and census information: visits, admissions, discharges, cost/visit, etc.

Responsibility: Provide care work services to clients as needed.

1. Act as an advocate for the client in meeting social and emotional needs.
2. Convey needs to family members or other departments and refer to other agencies as needed.
3. Inform clients of available community resources and referred as necessary.
4. Assist with any coordination of care or discharge planning for clients.

1. Participate in annual performance evaluation process.
2. Write yearly personal goals.
3. Participate in all required in-service programs at Bethesda.
4. Maintain a professional conduct and pleasant, friendly attitude towards team members, clients, and visitors.
