In this vital role, you'll combine your clinical background and relationship skills to ensure clients receive exceptional, personalized in-home care. Our Mission

- Meet with prospective clients and families to assess care needs and develop customized Plans of Care.
- Conduct service inquiries, care consultations, and quality assurance visits.
- Coordinate with staff and Care Professionals to ensure seamless, high-quality service.
- Assist clients in navigating long-term care insurance benefits.
- Build and maintain strong relationships with clients, families, and healthcare partners.
- Participate in an on-call rotation , including occasional weekends , to ensure continuity of care.
- CNA or other clinical experience strongly preferred .
- At least one year of experience working with older or dependent adults.
- Previous sales or strong customer background.
- Training, coaching or management experience desired.
- Excellent communication, organization, and problem-solving skills.
- Valid driver's license and auto insurance.
- Ability to pass a background check and drug screen.
- Proficiency in Microsoft Office; Salesforce experience a plus.
