PRN Physical Therapist (PT) – In-House Rehab Aventura at Carriage Inn – Dayton, OH Looking for a flexible PRN opportunity where you can focus on great patient care and be part of a supportive rehab team? Aventura at Carriage Inn is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team! This is a great opportunity for a PT who enjoys working with the skilled nursing and long-term care population and is looking for flexibility while making a meaningful impact on residents’ mobility, independence, and quality of life. What We Offer Flexible PRN scheduling In-house rehab environment Supportive and collaborative therapy team Competitive PRN rates Opportunity to provide skilled therapy to a diverse SNF/LTC population Net Health for therapy documentation PointClickCare (PCC) for facility documentation Responsibilities Evaluate residents and develop individualized physical therapy plans of care Provide skilled PT services to help residents improve strength, mobility, balance, and functional independence Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team Complete timely and accurate documentation Participate in discharge planning and resident/family education Maintain compliance with facility, state, federal, and professional standards Qualifications Graduate of an accredited Physical Therapy program Current Ohio Physical Therapist license in good standing SNF/LTC experience preferred Experience with Net Health and PointClickCare (PCC) preferred Strong communication and teamwork skills Dependable, compassionate, and resident-focused Aventura at Carriage Inn is an Equal Opportunity Employer. #J-18808-Ljbffr Aventura Health Group
