PHARMACIST – HOME INFUSION

Location: Columbus, OH
Salary: $128,000–$135,200
Experience: 2+ Years Acute Care OR 1+ Year Home Infusion

POSITION OVERVIEW

Our client is seeking a Pharmacist to provide clinical pharmacy services for patients receiving home infusion therapy.

You’ll oversee medication therapy, IV compounding, patient monitoring, education, and coordination of pharmacy services.

WHAT YOU’LL DO

• Evaluate patients and prescribed therapies for home infusion care.
• Develop individualized pharmacy care plans.
• Review labs, perform pharmacokinetic dosing, and make clinical recommendations.
• Dispense medications, supplies, and infusion equipment.
• Educate patients and caregivers on medications and infusion therapies.
• Monitor patient progress, labs, compliance, and treatment outcomes.
• Ensure compounding and clean-room activities meet USP <797> standards .
• Coordinate refills, supplies, labs, compounding, and deliveries.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Participate in on-call responsibilities as required.

WHAT YOU’LL BRING

• Active Ohio Registered Pharmacist license required.
• Bachelor of Science in Pharmacy required; PharmD or MS preferred.
• 2+ years of recent acute-care experience OR 1+ year of home infusion experience.
• Strong knowledge of IV admixture and intravenous therapy .
• Strong clinical judgment and medication-management skills.
• Ability to supervise pharmacy support staff.
• Strong communication, organization, and time-management skills.
