Work Environment\n

The role is fully remote, offering a flexible working environment ideal for balancing professional responsibilities with personal life. Our team operates Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a structured yet adaptable schedule. You will collaborate with a multidisciplinary team and utilize digital communication tools to maintain engagement and coordination with patients, facilitating assessments and consultations seamlessly.

Key Responsibilities\n

  • Manage a daily caseload of patients, providing tailored clinical social work services to facilitate treatment goals and support the mental health of patients and their families.

  • Conduct thorough assessments of patients’ social, environmental, and psychological factors that influence their health and provide appropriate resources and interventions.
  • \n
  • Engage in discharge planning activities, ensuring that patients are prepared with the necessary support systems for ongoing recovery outside the clinical environment.
  • \n
  • Document patient interactions and treatment plans accurately, maintaining a legal record of care to ensure accountability and quality of service.
  • \n
  • Participate in team meetings and case conferences to discuss patient progress and refine treatment strategies in collaboration with colleagues.

  • Assist in the development and implementation of community outreach programs aimed at raising awareness and providing education on mental health issues.

  • Perform other duties as assigned, maintaining flexibility in a dynamic healthcare setting.

Patient Population / Client Focus\n

In this role, you will primarily interact with diverse patient populations, which may include children, the elderly, and individuals experiencing emotional or behavioral challenges.   • A master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
