Registered Nurse, Field Nurse Case Supervisor Full-time Caring People is a family‑owned and operated licensed home care agency that has proudly served over 25,000 clients over the last two decades. The ideal candidate will have exceptional written & verbal communication skills, very organized strong clinical and supervisory experience, be technically savvy, demonstrate strong leadership and possess a high acumen in nursing specific to in‑home health care. You would be reporting to the VP of Nursing Services and are responsible for facilitating our client’s care. Responsibilities Travel throughout Bronx and Westchester NY to facilitate care for our clients, including open assessment and supervisory visits. Visit clients in their homes, hospitals, and rehab facilities; open new cases and supervise our aides. Establish employee/client assignments to provide adequate and appropriate client care. Develop a care plan for client(s) and assist staff in the implementation thereof. Review all admissions and implement the care plan, ensuring that all are within the scope of home health regulations. Accurately, timely, and ongoing evaluate and report client needs, services and outcomes (30/60 day supervisory visits). Act as liaison between client, family, physician, providers and facilities. Available for clinical on‑call responsibilities via phone per rotation. Benefits Company cell phone and laptop provided. Monthly Travel Stipend. 4 weeks PTO vacation (recurring annually). Minimum 2-3 years experience working in a home healthcare setting (preferred). Knowledge of applicable local, state, and federal laws of in‑home care. Must be very detail‑oriented and able to provide good follow‑through. Valid driver’s license and reliable transportation. BA degree preferred (not a mandatory requirement).
