Senior Program Director

Conducts and/or reviews performance evaluations of staff in accordance with policy. Monitors staff performance on regular basis and provides feedback. Meets regularly with facility directors and staff. Communicates regularly with the community. Monitors equipment and supplies. Reviews curriculum and service delivery model. Evaluates individuals referred and receiving services. Monitors implementation of programs. Coordinates and monitors the documentation process. Orients, trains and supervises Program Manager, Nurses, Supervisors, Direct Care Worker. Prepares staffing reports. Ensures and supervises the implementation of Person-Centered Plan. Participates in Interdisciplinary Team meetings. Assess and evaluate individuals strengths and needs. Advocates for individuals, makes program modifications as needed. Reviews, approves, signs, and dates Program Data Collection processes and assessments. Coordinates all services offered to each individual. Ensures individual exhibit signs of good personal hygiene. Investigates individual and staff incidents and complete required documentation. Ensures compliance with individual’s rights. Ensures individual ratio is maintained as established by residence. Participates in discharge planning. Knowledge & Skills Education: A degree in Human Services OR a bachelor’s degree in any related field with a minimum of 2 years of experience in Human Services. Experience: Supervisory experience in a human services setting. Certification: Must be credentialed or eligible as a Qualified Intellectual Disabilities Professional (QIDP), Qualified Mental Health Professional (QMHP).
