This role plays a key part in helping residents improve mobility, strength, and overall independence.### **Key Responsibilities*** Provide therapy treatments as directed by the Physical Therapist* Follow **physician treatment plans and care goals*** Document daily treatments and weekly progress notes accurately* Assist with **rehabilitation and care planning meetings*** Communicate patient progress with the interdisciplinary team* Assist with **discharge planning and caregiver education*** Maintain clean, safe therapy areas and equipment* Support training of restorative aides and nursing staff* Ensure compliance with **state practice act and facility policies**### **Qualifications*** Active **Physical Therapist Assistant (PTA) license in South Carolina** (in good standing)* Graduate of an accredited PTA program* Experience in **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba** preferred* Strong clinical, documentation, and communication skillsWe are an equal opportunity employer.
