Home health RN. Axxess software. Short visits. Easy start of care. healthy post-op ortho patients.

SOC $110

RV $70

1099 (preferred) & W2

No miles reimbursement

Responsibilities

• Assess and monitor patient health status, including vital signs and medical history.
• Develop and implement individualized care plans in collaboration with healthcare team members.
• Administer medications and treatments as prescribed by physicians.
• Provide education to patients and their families regarding health management and disease prevention.
• Perform case management duties to coordinate care across various settings.
• Utilize skills in trauma medicine, pain management, and home community care as needed.
• Engage in senior care practices to enhance the quality of life for elderly patients.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification required; Advanced certifications preferred.
• Home Care experience
• Strong interpersonal skills to communicate effectively with patients, families, and colleagues.
