Travel RN Case Manager for a 26-week assignment in Austin, TX. Provide nursing care and case management for residents with intellectual and developmental disabilities. Deliver individualized treatment plans while collaborating with interdisciplinary teams to improve outcomes.

Key Responsibilities:

• Provide comprehensive nursing care and support to residents with intellectual and developmental disabilities, ensuring individualized treatment plans are followed and adjusted as necessary.
• Collaborate with interdisciplinary teams, including physicians, families, and support staff, to coordinate services and align treatment goals.
• Perform detailed clinical assessments and monitor health trends to proactively address potential issues and improve patient outcomes.
• Maintain accurate and timely documentation of all healthcare interventions, assessments, and care plans within electronic medical records systems.
• Ensure compliance with federal and state regulations, including ICF-IID standards, and stay updated on relevant policies affecting care delivery.

Qualifications:

• Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
• Valid, unrestricted Texas RN license or license from a compact state.
• Basic Life Support (BLS) certification.
• 1-2 years of RN case management experience.
• Prior experience in long-term care or supportive living settings.
• Strong communication and interpersonal skills.
• Ability to collaborate effectively with interdisciplinary teams.
• Proficiency in electronic medical records (EMR) systems.

Preferred Skills:

• Advanced Cardiovascular Life Support (ACLS) certification (preferred).
• Experience working with individuals with intellectual and developmental disabilities (highly preferred).
