Benefits Medical: Multiple plan options Dental: Delta Dental or reimbursement account for flexible coverage Vision: Affordable plan with national network Pre‑Tax Savings: HSA and FSAs for eligible expenses Retirement: Competitive retirement package to secure your future Responsibilities Conduct patient visits (face‑to‑face and via telephone) Review patient health history and medication lists Administer immunizations Perform focused nutrition assessments and blood pressure checks Facilitate telehealth interactions, including gathering pre‑clinic information, researching prescription discrepancies, and assisting with prior‑authorization denials Collaborate daily with physicians, nursing colleagues, and support staff Provide nursing care under direct supervision of a registered nurse (RN) or other assigned supervisor Collect data through observation and communicate findings to assist RN or provider in patient assessment and care planning Participate as a member of the health‑care team and accept delegation for patient care planning as required Deliver care in accordance with the patient care plan, organizational policies, and principles of relationship‑based care Qualifications Graduate of an accredited school of practical nursing or passed LPN boards after an approved period of study One year of LPN experience within the last five years preferred; at least one year required if graduation was over two years ago Experience working in a team environment Computer skills required; proficiency with electronic medical record systems preferred Effective communication skills with the ability to build trust and rapport with team members Ability to work daytime hours with flexibility to meet staffing needs and adapt to unpredictable situations Ability to work effectively with multiple interdisciplinary care team members in a rapidly changing environment License and Certification Current RN license in the applicable state (required before start date) BLS (Basic Life Support) valid at least 90 days after start date; acceptable certifications include AHA, Heartcode®, Military Training Network, or Red Cross CPR/AED for Health Care Providers Additional state licensure(s) and/or specialty certification or training as required by the work area Compensation $24.07 - $34.95 per hour Schedule Full Time Hours per week: 48-80 Monday – Friday; variable start times, 8‑hour shifts No weekends Location Rochester, Minnesota Equal Opportunity Employer All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, gender identity, sexual orientation, national origin, protected veteran status, or disability status.
