Care Manager I, Adult (Cookeville, TN) Job Details Schedule: Mon‑Fri, 8:00a‑4:30p Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available. Job Summary Provide comprehensive care management services to adults with Severe Persistent Mental Illness (SPMI) and to children and youth with Severe Emotional Disorders (SED). Develop relationships with consumers and assist them in overcoming challenges to live healthy, successful lives in the community. Collaborate with an interdisciplinary team and community partners to offer innovative solutions that help consumers achieve their goals. Responsibilities Engage with consumers monthly to assess needs, develop care‑plan goals, and coordinate care. Complete visits with consumers in the community—home, school, court—to evaluate needs and connect where they are. Address mental health, physical health, social determinants of health, gaps in care, and ED/IP utilization. Provide a side‑by‑side approach to evaluate and address barriers to engagement. Assist with referral and linkage to community resources and advocate for consumers as needed. Collaborate with consumers’ support system to increase supports when necessary. Follow training handouts and policies to ensure quality care delivery. Maintain timely and comprehensive documentation per agency policies. Meet productivity standards as indicated in policies and procedures. Actively participate in daily team meetings prepared to share insight, offer solutions, and develop action plans. Follow up on plans developed in meetings. Participate in monthly supervision, discuss caseload, engage in professional development, and receive feedback. Develop and maintain collaborative relationships across departments and with community partners. Demonstrate the Integrated Care Core Values of connection, innovation, compassion, and integrity. Requirements Bachelor’s degree in a health‑related field of counseling, psychology, social work, or another behavioral health field. Applicants with a bachelor’s degree in criminal justice must have fifteen college‑level semester hours of coursework in behavioral health. Valid Tennessee Driver’s License. Acceptable Motor Vehicle Report (MVR). Acceptable Criminal Background Investigation. Personal Automobile Insurance.
