LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Chesapeake, Virginia.

40 hours per week
Shift: 8 hours, days
Employment Type: Travel

MUST HAVE recent experience in an adult acute hospital setting, with discharge planning!

5+ years of recent acute care case management experience (hospital setting)

o < 6 months gap from last acute care CM role
o Level I, II, or III Hospital for CM experience
o Census 15-25 for Care Management (typically indicates they are familiar with the model)
• 4/5 or 5/5 with all Care Management functions (including CMS regulations)
• Epic experience REQUIRED
• Experienced traveler (unable to consider first assignment at this time unless years of experience are extensive)
