This role supports individuals and families across the continuum of care. The RN utilizes the nursing process to assess, plan, implement, and evaluate care. They administer medications and treatments, perform clinical procedures, and provide patient education. Collaboration with providers and interdisciplinary teams is essential in care coordination. The RN ensures accurate documentation and adherence to infection control and regulatory standards. Efficient clinic operations are promoted while aiming for quality outcomes and patient safety. Culturally responsive care is delivered in a fast-paced ambulatory environment. Required certifications include Basic Life Support (BLS) and a preference for Case Management certification. A graduate of an accredited school of nursing is required, and a bachelor’s degree is preferred. High school diploma or equivalent is also required. Key Responsibilities include: Performing comprehensive patient assessments and developing individualized plans of care. Implementing and evaluating nursing interventions and patient responses. Administering medications and treatments safely and accurately as per provider orders and policy. Recognizing changes in patient condition and initiating appropriate interventions or escalation of care. Providing patient and family education related to care, medications, and discharge needs. Delivering culturally responsive, patient-centered care. The RN exercises independent nursing judgment in determining appropriate nursing actions and interventions. They develop, implement, evaluate, and revise individualized nursing plans based on patient needs. Clinical findings are integrated with ethical principles, cultural considerations, patient safety standards, and regulatory requirements. Accepting accountability for nursing decisions, delegated tasks, and outcomes of nursing care is vital. Collaboration with providers and interdisciplinary team members is essential for care coordination. Facilitating referrals, follow-up care, and continuity of services is part of the role. Documenting assessments, interventions, and outcomes accurately and timely in the electronic health record is required. Advocating for patient care and assisting patients in navigating the healthcare system are key responsibilities. The RN practices within nursing scope and adheres to regulatory, safety, and infection prevention requirements. Participation in quality and performance improvement activities is expected. Maintaining required competencies and certifications is necessary. Reporting adverse events, near misses, and unsafe conditions per policy is mandatory. Professionalism, accountability, flexibility, and teamwork are demonstrated daily. Guidance and delegation to ancillary staff are provided as needed. Supporting orientation, mentoring, and education of staff and peers is part of the role. The RN may float to other departments or assignments when necessary.
