Hospice Registered Nurse Responsibilities:

• Assess the physical, psychological, social, and spiritual needs of the patient and caregivers and formulate a comprehensive Interdisciplinary plan of care.
• Coordinate with physicians, caregivers, volunteers, ancillary therapies, and home health aides to support the care and needs of the patient.
• Educate on prescribed medications and supplies and ensure optimal quantities.
• Oversee and document home health aide plan of care every two weeks.
• Attend interdisciplinary team meetings to evaluate the plan of care and revise as needed
• Provide support at time of death and period of bereavement as needed.
• Document patient and family care plans, actions, goals, and concerns in Homecare Homebase (HCHB), AHN View email address on click.appcast.io's EMR, through a company-provided tablet.

Hospice Registered Nurse Qualifications:

• Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work.
• A minimum of one (1) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred.
• Must understand the hospice philosophy and team approach to hospice care
• Must be able to transfer patients as needed according to proper techniques of body mechanics to ensure patient and staff safety.
• Must have strong communication and organizational skills and high-standard of ethics and compliance.
• Ability to document within an electronic medical record.
• Associate Degree from an accredited School of Nursing or higher preferred.
