Job Summary

A mission-driven nonprofit organization in Pasadena, CA is seeking an Outreach Specialist to support individuals experiencing homelessness through street outreach, housing navigation, and case management services. This temp-to-hire opportunity is ideal for a compassionate, resourceful professional who is committed to helping vulnerable community members access housing, benefits, healthcare, transportation, and long-term stability.

The Outreach Specialist will work as part of a supportive Programs team with collaborative leadership, a strong community focus, and a shared commitment to meaningful client outcomes. This role offers the opportunity to build trusted relationships, coordinate critical resources, and directly contribute to helping individuals transition from homelessness into stable housing.

Key Responsibilities

- Conduct street outreach and engage individuals experiencing homelessness with professionalism, empathy, and consistency.
- Assess, screen, and triage client needs, referrals, and eligibility for housing and supportive services.
- Assist clients with obtaining required documentation for housing programs, benefits, and community resources.
- Connect clients to healthcare, mental health services, substance use treatment, employment support, public benefits, and transportation assistance.
- Support housing placement efforts, including permanent supportive housing and affordable housing programs.
- Maintain accurate case notes, HMIS documentation, and data entry while participating in case conferencing, team meetings, trainings, and community collaboration.

Compensation and Benefits

- Salary: $50,000 per year.
- Job Type: Temp-to-Hire.
- Schedule/Status: Full-Time, Non-Exempt.
- Location: Pasadena, CA.
