$70,000-$85,000 per year

Guided by our core values of Passion, Respect, and Excellence , the Wellness Coordinator oversees resident care services to ensure clinical quality, regulatory compliance, and a high standard of daily operations within the Resident Care Department.

Responsibilities for Wellness Coordinator (LPN):

• Oversee all areas of the Resident Care Department, including compliance, clinical planning, assessments, staffing, budgeting, and direct resident care.

• Ensure resident care needs are met and maintained in accordance with physician orders, state regulations, and company policies.

• Conduct and oversee resident assessments and ensure appropriate care plans are developed, implemented, and updated as needed.

• Provide clinical leadership, guidance, and support to nursing and care staff to ensure the delivery of high-quality care.

• Monitor staffing levels and shift schedules to ensure adequate coverage and efficient operations.

• Support regulatory compliance through ongoing audits, documentation review, and adherence to facility standards.

• Collaborate with interdisciplinary teams to ensure coordinated, resident-centered care.

• Promote a culture of accountability, teamwork, and continuous improvement within the department.

Qualifications for Wellness Coordinator (LPN):

• Graduate of an accredited nursing program with a current, active, and unencumbered LPN license in the state of Virginia .

• Minimum of 3 years of clinical experience in a long-term care setting required.

• Valid BLS Certification (CPR) required.

• Strong clinical assessment and critical thinking skills.

• Demonstrated ability to manage multiple priorities in a fast-paced healthcare environment.

• Experience in leadership, staffing coordination, or supervisory responsibilities preferred.
