Looking for an opportunity where you can build meaningful patient relationships and make a lasting impact every day? Blue Stone Therapy is seeking a Physical Therapist to join our rehabilitation team at Mount Olivet Care Home in Minneapolis !!

Working alongside an experienced interdisciplinary team, you'll provide individualized physical therapy services that help adult and geriatric patients regain mobility, improve function, and maximize their independence in a skilled nursing and post-acute rehabilitation setting.

What You'll Do

• Evaluate patients and develop individualized plans of care
• Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, pain management, and functional independence
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to achieve the best patient outcomes
• Supervise Physical Therapy Assistants in accordance with state licensure requirements
• Complete timely, accurate documentation supporting quality patient care and regulatory compliance

Requirements

Minimum Qualifications

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license (or eligibility for licensure) in Minnesota

Salary Description


Salary Range: $36.00 to $55.00/hour
