Physical Therapist - Full Time

We’re looking for a Physical Therapist to take on a full-time or part-time role with a skilled nursing facility in Sauk City, WI. Minimum Requirements Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA. Current Wisconsin Physical Therapist license. BLS certification from the American Heart Association or American Red Cross may be required. Experience working with adult and geriatric populations, preferably in a skilled nursing facility or long‑term care setting. Clinical documentation experience. Experience with Net Health is a plus. Your Responsibilities Provide physical therapy services to adult and geriatric residents. Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence. Support residents in achieving rehabilitation goals and milestones. Accurately document care and maintain compliance with facility standards. Collaborate with therapy and nursing teams to ensure patient‑centered outcomes. Educate residents and families on safe techniques and strategies to maximize independence. Participate in team meetings and contribute to care planning.
