Responsibilities
- Conduct initial evaluations and develop individualized PT treatment plans for residents in a geriatric care setting.
- Deliver evidence-based physical therapy interventions focused on mobility, strength, balance, gait, ROM, and Activities of Daily Living (ADL) training.
- Monitor and document patient progress, adjust treatment plans as needed, and communicate outcomes to the care team.
- Collaborate with nurses, therapists, and support staff to coordinate care and ensure patient safety during therapy sessions.
- Assist with discharge planning and progression toward functional goals, including patient and family education.

Requirements
- Current Physical Therapist license to practice in the United States; NH licensure or eligibility for licensure in New Hampshire may be required.
- Availability for a 13-week travel assignment with guaranteed 35 hours per week and daytime shifts (5x8).
- Ability to provide PT services in a geriatric/long-term care setting and document in patient records per regulatory standards.
- Strong communication and collaboration skills with an interdisciplinary team.
- Adaptability to a travel assignment and compliance with agency policies.
