Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting, ensuring their comfort and dignity. Monitor patient health by observing vital signs, administering medications as prescribed, and maintaining accurate care plans in compliance with HIPAA regulations. Support residents during transfers using equipment like Hoyer lifts and patient transfer methods to promote safety for both clients and staff. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to encourage proper nutrition. Engage in patient observation to identify changes in condition or behavior, especially for those with dementia or autism spectrum disorders. Maintain a clean and safe environment through regular cleaning routines and manual handling procedures aligned with health standards. Provide transportation assistance for appointments or community outings while ensuring client safety during travel. Document all care activities clearly and accurately, communicating effectively with team members about each resident’s status and progress. Qualifications Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient care techniques including medication administration, vital signs monitoring, and patient personal care assistance. Familiarity with specialized care such as Alzheimer’s care, memory care, dementia support, and hospice care is advantageous. Ability to perform manual handling tasks safely using equipment like Hoyer lifts; experience working with heavy lifting is a plus. Strong understanding of resident’s rights and confidentiality standards under HIPAA regulations. Excellent communication skills for engaging with clients effectively and documenting care accurately. Certification in CPR and First Aid is required; additional certifications related to healthcare or caregiving are beneficial.
