Job Summary With a commitment to patient- and family-centered care, the professional nurse uses the nursing process in ways that value the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health. The RN, Coordinator of Patient Care Services, assists in leading the daily operations of a specific unit, acts as a resource, lead, and clinical role model for the patient care team, advances and oversees patient care standards of the unit for the designated shift, and facilitates and coordinates patient care delivery and flow through the continuum of care. Supports nursing professional development, Magnet, and Shared Governance through participation in and mentoring of colleagues on councils and the Clinical Ladder program. Responsibilities Acts as a resource, role model, and clinical leader for the patient care team. Evaluates patient needs and makes care recommendations based on those needs. Monitors staff schedules to ensure adequate staffing to meet patient care needs and maintains the quality of patient care. Interacts with co‑workers, visitors, and other staff consistent with the values of Jefferson; recognizes patients, their family, or designated care partner(s) and provides safe, compassionate and coordinated care based on respect for personal preferences, priorities, needs, and cultural beliefs. Communicates with and supports the interprofessional teams and services, patient, family, and designated care partners; serves as a clinical resource regarding policies, procedures, patient standards, performance improvement, and risk management issues. Collaborates with the medical staff to facilitate consistent interdisciplinary plans of care to ensure quality patient care; assesses patient needs and makes care recommendations based on the patient's physical, psychosocial and cultural needs, age and functional status; facilitates timely patient throughput optimization for admissions, transfers, and discharge. Provides Smart Square schedules and on‑call schedules; follows up and resolves customer and physician issues and complaints. Authorizes overtime or scheduling changes as necessary; collaborates with the Nurse Manager to evaluate, maintain and improve team member performance; assists in implementing changes in clinical practice and evaluates the quality and effectiveness of nursing practice. Supports nursing professional development, Magnet, and Shared Governance through participation in councils and mentoring; provides patient care support when necessary. Minimum Qualifications Specialized Diploma Graduate of an accredited school of nursing required. Three years as a Registered Nurse in an acute care setting. Effective communication skills with the ability to actively listen, transmit, receive, and accurately interpret ideas, information, and needs. Patient‑and‑Family‑Centered practice with knowledge of how to convey respect and collaborate with patients and families. Cultural awareness and the ability to connect effectively with diverse backgrounds. Care coordination knowledge of multidisciplinary services and ability to integrate treatment for optimal patient care. Patient safety knowledge and ability to reduce risks faced by patients. Medication administration and prescription knowledge of medications used in patient care. Discharge planning knowledge of patients’ post‑discharge needs and follow‑up care. Assessment and diagnosis knowledge and ability to analyze causes behind symptoms. Health information documentation knowledge and ability to record medical histories, vital signs, medications, diagnoses, and treatment plans. Basic Life Support (BLS) – American Heart Association. RN license in PA, NJ, or other applicable states. Preferred Qualifications Bachelor’s Degree (BSN) or completion of BSN/advanced degree within three years of hire. Certification in specialty relevant to the unit; if required, must be obtained within six months of hire date. Physical Demands Lift and carry 40–50 lbs. Push or pull patients on bed or stretcher (requires ~29 lbs. push force). Perform lateral transfers up to 50 lbs. of patient weight. Frequent to continuous standing, walking; may walk 8–10 miles per shift. May require kneeling, bending, squatting, and performing CPR.
