Physical Therapist

Provide direct patient care activities to patients as directed by physician orders including assessment, treatment planning, implementation, evaluation, and instruction/supervision of physical therapy practice within the Standards of Practice for Physical Therapy, and in compliance with the applicable laws, regulations, and agency policies. Perform the initial and/or comprehensive physical therapy evaluation visit, identifying the primary focus of care, developing an individualized plan of care in collaboration with other members of the interprofessional or interdisciplinary team, patient, family, or caregiver. Educate patients and their families or caregivers about managing patient’s illness or injury, including home care needs, home safety, and use of assistive and adaptive devices. Participate in care coordination and discharge planning activities and acting as a resource to other health care personnel in meeting patient's needs. Participate in activities that promote patient safety, quality, and regulatory compliance. Participate in professional development and lifelong learning activities. Supervise the PTAs. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to 50 pounds independently; occasionally lift and/or move up to 200 pounds with assistance. Other duties as assigned. Compensation & Benefits The pay range for this position is $57-$64/hour, depending on experience and qualifications. Bachelor's, Master’s, or Doctorate degree in Physical Therapy Current Physical Therapy license in the state(s) of practice Current Basic Life Support/CPR certification Current and valid driver’s license, reliable transportation, and current liability auto insurance. Ability to work well with others and take direction from management, as well as take initiative – willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment. Self-motivated to accomplish identified goals with a strong sense of accountability for results. Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable. Work for this position is performed in patient homes, care settings/facilities, and office settings. This position will have to navigate through a variety of different settings to provide care. There is exposure to varied home environments and potential infectious conditions. Must comply with all Dover Health and safety requirements.
