Licensed Practical Nurse (LPN) in Home Health

Provides coordination and direction for home nursing care according to physician's orders. Responsible for following agency policies and procedures through the competent application of the nursing process. Reports directly to the Clinical Manager. Supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits are required. Periodic office meetings will be held. Must have reliable transportation. Current driver's license and agency-required liability insurance are necessary. Average number of patients per day is 5-8. Each type of visit and activity holds a different point value. Contractor will be assigned between 5-10 patients per day. Please reach out to the manager if unable to meet financial needs due to low patient census. Types of cases include Start of Care, Wound Care, and Follow-up Visits. No first-timers accepted; recent specialty experience is required. Weekend requirements vary by location and profession; specifics to be discussed during the interview. The territory mileage radius requires willingness to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. Local candidates are accepted at the same rate. All documentation must be completed within 24 hours of patient appointments. Any delayed or incomplete documentation that impacts billing will be addressed. There are no guaranteed hours with this contract. Weekly productivity points of 30 are expected, equating to approximately 40 hours per week. On-call rotation is required, specifics will vary. All travelers must be proficient in the required EMR systems and complete training if necessary.
