Registered Nurse (RN) - Heart Failure Clinic

Job Overview Department: 02050 AMG Highway 50 – Cardiology. Status: Full time . Hours per week: 36. Schedule: Monday‑Friday 8:00AM – 4:30PM, off every other Friday. Travel required to the Mount Pleasant location at least one day per week. Pay Range: $41.10 – $61.65 per hour. Major Responsibilities Provides continuity of care by ensuring smooth transitions between care settings. Develops a relationship with the patient and their multidisciplinary team to facilitate navigation through treatment, follow‑up, and subsequent care. Completes a nursing assessment to identify the patient’s individualized needs and collaborates with the multidisciplinary team to develop an interdisciplinary plan of care. Supports the patient and family through education, addressing psychosocial needs, advocacy, and identification of barriers to care. Navigates patients and family members throughout diagnosis, treatment and follow‑up stages, ensuring continuity of care. Provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning. Coordinates timely, seamless, evidence‑based care across the care continuum to achieve optimal patient outcomes. Evaluates, selects, or designs patient education materials related to specific disease site programs. Provides education and outreach programs to patients, Aurora staff and community members. Assists leadership in developing program vision, goals, and objectives. Supports the development, implementation, and maintenance of quality programs that promote best practice patient care outcomes, quality of life, and enhanced patient loyalty. Education and Certification Bachelor’s Degree in Nursing. Registered Nurse license issued by the state in which the nurse practices. Heart Failure (CHFN) Certification from Cardiovascular Nursing Education Associates (CNEA) required within 2 years. Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) required within 6 months unless exempt. Experience Required 5 years of experience in cardiology nursing, including nursing assessments, care plan development, and patient education. Knowledge, Skills & Abilities Working knowledge of quality and acute patient care needs related to heart failure. Maintains clinical expertise to manage physical, emotional, psychosocial, and spiritual needs of patients throughout the care continuum. Excellent analytical and interpersonal communication skills for negotiation with families, patients, physicians and third‑party payers. Demonstrated ability to work well with physicians and other professionals in a direct and positive manner. Physical Requirements and Working Conditions Clear oral communication and hearing acuity required for receiving instructions and conversing on standard telephone. Functional speech and hearing to allow effective communication over the telephone. Exposed to a normal office environment. Operates all equipment necessary to perform the job.
