Patient Navigation services will support CHN to better serve those disproportionately at risk for or living with HIV/HCV by improving their access to mental and behavioral care.Essential Job Functions:Provide screenings, outreach and initiatives to promote clinical programingCoordinate and maintain the behavioral health waitlistProvide appropriate patient referrals to both internal and external agenciesDetermine eligibility of services for Behavioral Health patient as they are referredScreen and Scheduling Behavioral Health outpatient sessionsProvide psychoeducation as neededAssess for mental health needs, acuity, and appropriate placementLiaison between patient, internal systems and external insurance providerNavigating billing landscape and sharing knowledge with providersPerform mental health assessments, create plans, and assist patients with access to care, medication and adherence to treatment as well as risk reduction plansResponsible for tracking, monitoring and occasional reporting of patient activities, including access adherenceMaintain accuracy and timeliness of laws/policies impacting behavioral health at CHNComplete necessary paperwork and data entry in a timely fashion including entering case notesCollect necessary documentation required for program complianceAssist patients as they move through the service delivery systemAssist with the elimination of barriers including, but not limited to: insurance, financial, housing and transportationProvide emotional support for patients and their families, as neededAct as a liaison between patient and all relevant professionals, programs, and resourcesEducate patients and others about HIV disease process and preventionAssist with, and coordinate, community education and outreach initiatives as requestedMaintain and upkeep of electronic health records (EHR)Participate and attend staff, department, team and community meetings upon requestPerform effectively within the organization and interpersonally with co-workersDevelop positive working relationships with other social service, medical and behavioral health providersParticipate in fund-raising and advocacy eventsMust be available to work evenings and weekendsMust have current and valid Colorado driver’s license; must be able to pass driving record check per CHN insurance requirements.Must be able to be insured by CHN driving and liability insurance companies.Must have personal insurance on own vehicle if driving on behalf of CHN.Must be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities.Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues. Education and other experiences may fulfill this requirement. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.Education:Bachelors or Masters Degree in social work, sociology, psychology, counseling, public health, or nursing; And/or equivalent combination of education and experience required.Software Utilized:Proficiency with Microsoft Office applications such as Word, Excel, Outlook, and Teams is required. Candidates should be comfortable working with client database systems and learning new software tools as needed.Knowledge, Skill and Ability:Mental health and harm-reduction knowledge. Basic knowledge of HIV, STIs, PrEP and other prevention methods. Proactive problem solver who works well in multi-disciplinary settings. Ability to communicate well with medical providers and support staff. Strong verbal and written communication skills. Strong socio-cultural identification or work experience with MSM and LGBTQ+ preferred. Comfort discussing sexual health and risk reduction strategies. Proven organizational skills with high attention to detail. Ability to work well with diverse populations. Ability to work well within the organizational structure. Previous experience in the delivery of evidenced-based programs is highly preferred, as well as excellent communication, client services, and data collection skills. The appropriate person will be highly organized and detail-oriented. This position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics, and cultural competency. Must demonstrate accuracy, efficiency and superior organizational skills. Must demonstrate effectiveness in the following areas: task and time management; teamwork; and communication with teams, managers, clients and participants, and company consultants.Ability to speak Spanish desired.Material and Equipment Directly Used:Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine. Works with word processing software, spreadsheet applications, and presentation software. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax.Physical Demands and Work Conditions:Works in an office environment. May travel via automobile or airplane to offsite locations or for CHN business. Must have and maintain a valid Colorado driver’s license and auto insurance on personal vehicle. Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding.
