Supported Employment Program Case ManagerOrganization Overview:Community Services League (CSL), founded in 1916, is one of the Kansas City region’s largest social service providers. CSL’s commitment to assist families from crisis to permanent stability is a critical pillar within the organization’s mission and vision.Position OverviewThe Supported Employment Program Case Manager at Community Services League (CSL) is responsible for the oversight of CSL's Litter Crew crew members as it relates to permanent employment, housing and successful graduation from the program.ResponsibilitiesThe chosen candidate reports to CSL's Senior Vice President of Housing Services and will provide the following services and management oversight, and any other duties as assigned by the supervisor:Client Assistance: Helping crew members achieve economic stability through needs identification, program assistance, support, referrals, training, or other interventions, initiatives, or efforts using directive and supportive behavior.Assist with the coordination of services for each crew member to include housing services, career services, vocational rehabilitation and financial coaching.Transport clients to and from appointments with other departments within CSL.Help facilitate needs assessments for litter crew members.Coordinate continuity of services with collateral agencies and coordinate services in the community.Develop and revise client goals regularly to ensure clients are maintaining their housing, employment and financial outcomes.Data & Documentation: Verify to those who support us how we use resources to achieve economic stability -accounting for the use of resources per client, projecting future client needs, and managing resources to maximize the impact for our fellow citizens and stakeholders.Prepare all necessary and required forms and documents to present for approval and implementation of financial assistance to clients.Develop and maintain client files with all documents verifying program requirements have been met.Maintain accurate and up-to date client charts, recording progress made, and services provided and entering clients in our HMIS system as well as Show the Way in a timely manner, along with internal data collection systems.OutreachAssist with the coordination of homeless outreach activities.Assist in screening outreach clients for eligibility when pulled from the by-name list for housing referral.Assist in the selection, evaluation, and supervision of clients who qualify for services utilizing Coordinated Assessment through the Continuum of Care (CoC).Coordinate continuity of services with collateral agencies and coordinate services in the community.Hours/Schedule: The typical hours for this position are 8am-5pm, Monday-Thursday and 8am-12pm on Friday. From time to time, this schedule will vary depending on the needs of clients, partners, or programs.Salary/Benefits:Salary will be commensurate with experience with a salary range of $46,988 - $58.306. CSL offers an employee health, dental and vision plans with eligibility beginning the first of the month following 30 days of employment. CSL also offers a retirement savings plan with company contributions.
