What You'll Do

• Monitor residents' physical and mental well-being continuously; report significant changes to supervisors
• Accurately transcribe and implement physician orders
• Administer medications and treatments per physician instructions, document all care accurately
• Supervise Licensed Nursing Assistants (LNAs), ensuring high-quality resident care
• Assist physicians during resident rounds and facilitate communication regarding medical needs
• Perform wound care, administer oxygen, and assist with gastrostomy/jejunostomy feedings
• Support infection control practices and ensure unit cleanliness and safety
• Manage resident admissions, discharges, transfers, and documentation
• Maintain communication with residents' families regarding care and consent
• Assume responsibilities for the unit during shifts under supervision
• Follow all federal and state regulations governing medication storage and documentation
• Assist with staff orientation and ongoing education for nursing team members
• Support and evaluate nursing staff performance as per facility policy

What You Need

• Valid Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in New Hampshire
• Prior experience in long-term care or similar setting preferred
• Strong assessment and communication skills
• Ability to work independently and as part of a team
• Compassionate, detail-oriented, with a focus on resident safety and comfort
