Speech-Language Pathologist (SLP) - Full-Time | Split Position

Location: Two Rivers & Kewaunee, WI
Facilities: Hamilton Health Services and Kewaunee Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) for a full-time split position serving Hamilton Health Services in Two Rivers, Wisconsin, and Kewaunee Health Services in Kewaunee, Wisconsin .

As a Speech-Language Pathologist (SLP) , you will work with adult and geriatric patients to improve communication, cognition, swallowing function, and overall quality of life while collaborating with an interdisciplinary rehabilitation team focused on excellent patient care.

• Evaluate and treat patients with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized speech-language therapy treatment plans.
• Provide skilled interventions related to communication, cognition, voice, and dysphagia management.
• Complete assessments and make recommendations related to swallowing safety and functional communication.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Educate patients, families, and staff on treatment strategies and safety recommendations.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Deliver compassionate, evidence-based speech-language pathology services.

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Wisconsin Speech-Language Pathologist (SLP) license required.
• ASHA Certificate of Clinical Competence (CCC-SLP) required based on state regulations.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates and Clinical Fellows are encouraged to apply.
