RN - Case Manager

Case Manager Registered NurseThis position requires a registered nurse specializing in case management. The role involves actively participating in discharge planning and care coordination. You will organize and conduct family meetings to discuss treatment plans. Experience with various patient demographics including neonates, adolescents, adults, and geriatrics is necessary. The daily patient census is approximately 25. You will work closely with interdisciplinary support teams including physical therapy and social services. This role demands seasoned travel experience, without floating expectations. Weekend shifts are required to meet departmental needs. Make sure to request time off upfront as changes will not be accommodated post-submission.Minimum of 2 years in acute care case management/discharge planning required. Current state license required (pending license accepted but must be licensed upon start date). Valid state compact license must be in hand at submission.
