Facility: Lake Eustis Healthcare & Rehabilitation
Location: Eustis, FL
Schedule: Full-Time | 8:00 AM – 4:30 PM
Salary: $80,000 – $110,000 annually, based on experience and licensure

Join Our Team

Lake Eustis Healthcare & Rehabilitation is seeking an experienced Rehabilitation Director to lead and oversee our rehabilitation department. The ideal candidate will have strong clinical and leadership experience and a passion for delivering high-quality outcomes for residents.

Key Responsibilities

• Direct and oversee daily rehabilitation services and department operations.

• Ensure compliance with federal and state regulations and professional standards.

• Develop individualized rehabilitation programs in collaboration with residents, families, and the interdisciplinary team.

• Manage staffing, scheduling, resources, and departmental performance.

• Build strong relationships with physicians, healthcare providers, and community partners.

• Monitor documentation and ensure accurate, timely, and thorough therapy records.

• Lead interdisciplinary meetings and support positive resident outcomes.

• Identify departmental challenges and implement effective solutions.

• Maintain survey readiness and ensure compliance with rehabilitation standards during inspections.

• Report departmental performance and needs to the Administrator.

Qualifications

• Bachelor’s degree in Rehabilitation or a related field required.

• Current, active, and unencumbered therapy license.

• Open to Physical Therapy, Occupational Therapy, or Speech Therapy professionals.

• Minimum 3 years of rehabilitation experience .

• At least 1 year of healthcare supervisory or leadership experience , preferably in a SNF or hospital setting.

• Strong knowledge of RUGs and CMI .

• Prior SNF rehabilitation leadership experience strongly preferred .
