We're looking for passionate Registered Nurses (RNs) to join our dynamic team and contribute to our mission of enhancing the well-being of our patients.Home Health Registered Nurse (RN) responsibilities include:providing services in accordance with the plan of caremaking the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisionsmonitoring and evaluating patients' progress and updating care plans accordinglyeducating patients and their families on managing the conditions and promoting wellnessinforming physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible caretaking on-call duty nights, weekends, and holidays, as assignedparticipating in in-service programs, supervising and teaching other nursing personnel as necessaryRequirements for the Home Healthcare Registered Nurse position:valid Registered Nurse (RN) license in the state of Illinoisminimum of one year of nursing experience, preferably in home health or a related settingproficient in using electronic medical record systems, such as Axxessprevious experience in geriatrics and home healthability to work independently and prioritize tasks effectivelyability to provide care with empathy, respect, and understandingstrong communication and interpersonal skillsJob Type: Per DiemPay: Varies.
