PanCare of Florida is seeking a skilled and compassionate Diabetes Care and Education Specialist (DCES) to join our team. In this position, you will empower individuals with the skills, tools, and confidence needed to manage diabetes and make long-term lifestyle changes. You will work closely with patients, caregivers, and the healthcare team. You will be at the forefront of ensuring the safe and effective use of medications, working closely with healthcare professionals and patients to optimize treatment outcomes. Your responsibilities include the following:

• Conducting Diabetes Care and Education (DCES) participant assessments using the AADE7 framework
• Developing measurable goals, teaching plans, and individualized learning objectives
• Providing education using interactive and skill-based methods to increase engagement and understanding
• Documenting patient assessments, learning plans, progress, and evaluations outcomes
• Performing follow-up assessments and track participant progress toward defined goals
• Referring patients to appropriate health providers as needed
• Communicating relevant findings and updates to the primary care provider and healthcare team
• Participating in developing program and teaching materials
• Supporting program accreditation requirements and participate in accreditation surveys
• Engaging in continuous quality improvement initiatives for Diabetes Care and Education Specialist (DCES)
• Maintaining required annual continuing education specific to diabetes and behavioral change strategies
• Staying current with advancements in diabetes management, technology and educational best practices

Required Qualifications:

• Active RN License
• At least 1 year experience with DCES programs
• At least 1 year experience developing therapeutic relationships and motivating participants
• Strong understanding of diabetes treatment and education
• Ability to present education content
• Strong communication and teaching skills

Preferred Qualifications:

• Certified Diabetes Educator (CDE) or experience working in diabetes education preferred
• At least 3 years experience with DCES programs
• At least 3 years experience developing therapeutic relationships and motivating participants
