Foster Care Care Coordination Specialist

EmblemHealth is seeking an experienced administrative support professional to assist the Foster Care program. You will coordinate with OCFS, LDSS, and VFCA to ensure timely assessments and continuous care for members in foster care, acting as a subject matter expert on trauma-informed foster care healthcare needs.
The role requires a bachelor’s degree, NY RN license and NYSW licensure; CCM certification within 180 days or equivalence.
