Consultant Pharmacist

Overview

Presbyterian Homes & Services is seeking aConsultant Pharmacistto join its Radius Living Rx team.

The Consultant Pharmacist collaborates directly with the Director of Clinical Services and Radius client facilities to enhance patient health by ensuring optimized medication regimens. This role completes drug regimen reviews, provides subject matter expertise to facilities, patients, and families, identifies and addresses areas for improvement, and assists facilities with compliance with applicable laws and regulations. The Staff Pharmacist reports to the Director of Clinical Services.

Radius Living Rxis a long-term care pharmacy serving residents in partner communities across Minnesota and Wisconsin. Our pharmacists, technicians, and support staff work closely with facility care teams and prescribers, so the work stays close to the people it's for. We're building a team that wants pharmacy practice grounded in that relationship: careful medication management, direct communication with facility staff, and a genuine stake in how residents are doing.

Responsibilities

• Perform medication drug regimen reviews as outlined by regulation or contract for facility type, and provide written reports of these reviews.

• Collaborate with physicians and other health care providers in managing medication therapy to ensure patient needs are met, providing recommendations to optimize pharmacotherapy.

• Participate in client QAPI meetings, antibiotic stewardship meetings, and psychoactive therapy meetings.

• Maintain accurate documentation of clinical interventions and consultations.

• Support clinical initiatives in the long term care space, including chronic care management, comprehensive medication reviews, and other medication management services.

• Support facilities in remaining compliant and aligned with federal regulations.

• Educate facility staff on medication related topics and best practices, including in-service training activities related to medication administration and medication therapy.

• Demonstrate good stewardship of available resources. Assist in the care and maintenance of departmental equipment, supplies, work areas, and facilities; obtain supplies and equipment as needed.

• Promote continuous self-development and growth. Maintain current knowledge of community resources and organization and industry policies and practices. Use discretion and independent judgment to compare and evaluate possible courses of action and decision making.

• Support the integration of Christian Culture through communication and education systems.

• Other areas as identified to support aligned goal setting and achievement as driven by the Director of Clinical Services.

Qualifications

• B.S. in Pharmacy or Pharm.D. degree required.

• Active pharmacist license in the State of Minnesota; Wisconsin licensure preferred.

• Three years of experience in long term care pharmacy practice preferred.

• Ability to visit long term care facilities across Minnesota and western Wisconsin. Valid U.S. driver license required.

• Strong clinical knowledge and excellent customer service and communication skills.

• Proficiency with standard email and Microsoft products.

• Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.

• Demonstrated compatibility with PHS’ mission and operating philosophies.

Pay Range

The estimated starting pay range for this role is $130,000 - $150,000 per year. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
