RN Case Manager: Care Coordination & Discharge Planning

LanceSoft is seeking an experienced Registered Nurse Case Manager (RN) to coordinate patient care, discharge planning, and resource utilization while collaborating with multidisciplinary teams to ensure quality outcomes. The role requires an active Nevada RN license, BLS, and at least 3 years of acute care nursing with 1+ year in case management or discharge planning.
