Registered Nurse RN Case Manager Home HealthImmediate opening for a Registered Nurse RN Case Manager Home Health in Scranton, PA and surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.Pay and Benefits$38–$40 per hourWeekly pay with direct depositMileage reimbursementPTO and paid holidaysHealth, Dental, AFLAC insurance, Retirement Plan (company match up to 3%)ScheduleMonday through Friday 8:30 AM to 5:00 PMOn-call, weekend, and holiday rotationRegistered Nurse RN Case Manager ResponsibilitiesManage a home health caseload in Scranton, PA and surrounding areasPerform RN assessments and reassessmentsDevelop and update care plansCoordinate care with physicians and healthcare teamComplete home health documentation and ensure complianceEducate patients and familiesRequirementsActive Pennsylvania Registered Nurse RN licenseMinimum 1 year home health RN experienceCase Manager or Case Management experience preferredCPR certificationValid driver’s license and reliable transportationAbout Interim HealthCare of ScrantonInterim HealthCare of Scranton is a locally owned home health agency serving Scranton, Lackawanna County and surrounding NEPA communities. We provide consistent patient assignments, manageable caseloads, and strong clinical support for our Registered Nurse RN team.
