Job Summary and Qualifications

In this role, you will be responsible for delivering direct patient care in alignment with physician directives, adhering to established standards of practice, and following organizational policies and procedures. You will implement the care plan, administer treatments, and collaborate with team members to achieve positive patient outcomes.

Your responsibilities will include:

• Adhering to the care plan, providing treatments, and collaborating with team members to ensure positive patient outcomes.
• Delivering direct patient care as outlined in the State Nurse Practice Act, including specialized tasks related to end-of-life care.
• Coordinating hospice services and effectively communicating identified needs to the interdisciplinary team, including Hospice Aides, the Director of Clinical Services – Hospice, and Chaplains.
• Managing pain and symptoms appropriately.

Qualifications required for this position:

• Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
• Valid Class C License.
• Registered Nurse credential.
• Associate Degree in nursing.
