About the Position

• Provide skilled nursing care and case management to patients in their homes.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Monitor patient conditions and respond to changes in health status.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Educate patients and families on disease management, medications, and treatment plans.
• Supervise Home Health/Hospice Aides and CNAs to ensure quality patient care.
• Maintain accurate documentation in compliance with OASIS and agency standards.
• Participate in on-call rotation and provide home visits as needed.

Requirements

• Active  Montana RN License (Required)
• BSN preferred
• Valid driver's license, reliable transportation, and current auto insurance
• Minimum 2 years of recent acute care, home health, or hospice experience preferred
• Experience with venipuncture, catheterization, wound care, and/or parenteral therapy preferred
• Strong assessment, communication, and case management skills
