RN Case Manager - Sign-on Bonus - $7,500

Full‑time RN Case Manager position located on the north side of Greenwood, offering a $7,500 sign‑on bonus and flexible work/life balance. Benefits include competitive pay, daily pay option, no overtime required, and direct 1:1 patient care. Responsibilities Conduct in‑person patient interviews and comprehensive physical assessments. Oversee and assess the implementation of patients’ plans of care, managing home‑health aides, LPNs, RNs, and other caregivers. Communicate patient status and collaborate with providers to adapt care as needs evolve. Educate patients and families on home‑health procedures, including wound care, IV administration, and medication management. Reduce readmissions by promoting preventive care and ensuring continuity of care. Qualifications Current unrestricted RN license in the state of employment. CPR/AED/BLS/First Aid certification. Reliable transportation to care sites. At least one year of RN experience in home‑health or a similar setting (case‑management or utilization review experience preferred). OASIS experience preferred.
