Spectrum Healthcare Resources has an excellent opportunity for a civilian Full-time Clinical Nurse Specialist at Vance Air Force Base in Enid Oklahoma .

Requirements:

• Education: Associate or Bachelors degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) the Commission on Collegiate Nursing Education (CCNE)
• Experience: ADN- At least 3 years experience in nursing and care coordination
• BSN- At least 2 years experience in nursing and care coordination
• Licensure: Maintain an active valid current and unrestricted license (with no limitations stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction
• Current Basic Life Support Certification

Responsibilities:

• The Special Needs Coordinor oversees and manages clinical procedures for family members that have special medical and educational needs.
• Collaborate with a multi-disciplinary group and other key service providers such as installation youth and childcare facilities and office and enlisted groups to ensure effective outreach and identification of special needs.
• Find the most appropriate medical resources and providing optimal care for the patient
• Handle case assignments review case progress and determine case closure
• Help clients achieve wellness and autonomy
• Interact with clients and develop effective working relations with medical team
• Help clients with continuity of care for specialty services and home health needs
