CareRite Centers seeks a Wound Care LPN to provide specialized wound management for post-acute and long-term care residents. In this role, you will assess, stage, and document wounds; perform evidence-based treatments; monitor healing progress; and collaborate with the wound nurse, physicians, therapy, and nursing teams to develop and adjust care plans. You will educate residents, families, and staff on prevention, skin integrity, and pressure injury reduction while maintaining strict infection control and regulatory standards in a fast-paced, resident-centered environment.
