Hospice RN Case Manager - Howard County

Job Overview The Hospice Home Care Nurse reports to the Team Manager and is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for terminally ill patients and their families. Qualifications Bachelor’s degree (BSN) or Associate or Diploma in Nursing, from an accredited institution. Registered Nurse licensure in Maryland (current). Minimum two years of nursing experience, or a comparable combination of experience and education. Skills Knowledgeable of universal precautions for infection control and safe nursing standards for self‑protection. Strong time management skills. Excellent interpersonal and communication skills. Patient and family‑focused orientation. Proficient use of an electronic medical record. Principal Duties and Responsibilities Complete initial assessment of patient and family to ascertain home‑care needs. Prepare and implement the care plan in collaboration with the interdisciplinary group. Provide direct nursing care to maintain the highest level of care. Plan and supervise the delivery of care by the home health aide. Cooperate with the Clinical Manager in utilizing other team members, outside agencies, and community resources. Update the primary physician monthly or as needed and attend interdisciplinary team conferences. Maintain problem‑oriented charting for continuity of care. Teach family members, facility staff, and caregivers skills such as pain control, medication titration, injections, draws, IV management, dressing changes, hygiene, nutrition and vital‑sign checks. Ensure necessary equipment arrangements. Attend to patient, family and facility at the time of death. Facility Specific Job Functions Document in facility chart. Maintain positive relationships with facility staff. Reconcile medications against facility medication list. Update facility staff and family on visit schedule. Attend care‑plan meetings and patient‑care rounds for hospice patients in the facility. Establish and maintain professional relationships with a variety of facility staff. Provide educational in‑services to facility staff. Participate in marketing and customer‑service activities with BDMs. Pay Range $66,942.34 – $102,872.41, with final salary based on qualifications, experience and alignment with organizational needs.
