Nurse Practitioner

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities Provide effective, equitable, value-based primary care to adult Medicare patients Conduct office visits for routine and acute issues, and administer wellness visits and health risk assessments Coordinate care with other providers, specialists, and testing facilities Lead population health initiatives and ensure preventive screenings Assist with phone triage, patient education, and outreach activities Conduct home visits as needed and participate in promotional activities Qualifications & Skills Master of Science in Nursing, Doctor of Nursing Practice, or NP certification Active, non-probationary state Nurse Practitioner license and DEA license National certification in Family, Adult-Gero, Adult, or Gerontological Nurse Practitioner Experience in primary care, understanding of HCC documentation, ICD-10 coding, and HRAs (preferred) Strong teamwork, communication, and patient relationship skills; bilingual abilities a plus Additional Details Full-time role with approximately 40 hours per week Competitive pay range and comprehensive benefits, including medical, dental, vision, retirement, and wellness programs Works closely with a collaborative care team, reporting to the Center Medical Director #J-18808-Ljbffr Preferred Home Health Care & Nursing Services
