RN Case Manger, Per Diem

About This Role The RN Case Manager, Per Diem at Hackensack Meridian Health is responsible for coordinating patient care activities to ensure compliance with evidence-based practice standards and regulatory requirements. This role is ideal for a skilled nurse who excels in collaboration and communication within a multidisciplinary team. Highlights Location: Hackensack, NJ Schedule: Per Diem, Rotating What You'll Do Facilitate collaborative management of patient care across the continuum. Apply process improvement methodologies in evaluating outcomes of care. Coordinate communication with physicians, nursing, and ancillary staff. Evaluate care provided against the Length of Stay. Collaborate with Admitting to ensure accuracy of patient demographic and insurance information. Communicate with patients and families regarding medical and discharge plans. Assess and plan for discharge needs and arrangements. Complete utilization management for assigned patients. Monitor length of stay and clinical resource use on an ongoing basis. Manage all aspects of discharge planning for assigned patients. Requirements Education Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees. Experience At least 2 years clinical experience in the relevant clinical specialty preferred. Licenses & Certifications Current NJ State Professional Registered Nurse License required. BLS AHA Basic Health Care Life Support HCP Certification required.
